Telehealth Informed Consent

Telehealth involves the use of electronic communications, including audio-video interface such as videoconferencing, to access health services remotely. By consenting to telehealth services through our Platform, you acknowledge and agree to the following:

Expected Benefits

• Improved access to medical care by enabling you to remain at your location while your clinician consults with you remotely
• More efficient care evaluation and management
• Obtaining the expertise of a specialist as needed

Possible Risks

As with any medical procedure, there are potential risks associated with telehealth, including but not limited to:

• Information transmitted may not be sufficient (e.g., poor resolution of images) to allow for appropriate medical decision-making by the clinician
• Delays in evaluation or treatment could occur due to deficiencies or failures of the equipment or technology
• In very rare instances, security protocols could fail, causing a breach of privacy of personal medical information
• A lack of access to complete medical records may result in adverse drug interactions or allergic reactions, or other judgment errors

Your Rights

You may withhold or withdraw your consent to telehealth at any time without affecting your right to future care or treatment. You understand that you may still receive care through an in-person visit with a local healthcare provider.

HIPAA Compliance

We are committed to protecting your health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA). Your confidential health information will be handled as follows:

• Your health information will remain confidential except when necessary for the provision of your care
• We may contact you by telephone, e-mail, or U.S. mail to provide appointment reminders and health-related information
• Your information may be shared with healthcare providers and insurance payers as needed for treatment, payment, and healthcare operations
• Your confidential information will not be used for the purposes of marketing or advertising

Financial Agreement and Shipping Terms

By using our services, you authorize charges to your credit card or other payment method for services rendered. You agree to the following terms:

• Payment is required at the time of service
• You authorize charges for the services and medications prescribed
• Medications are considered dispensed when signed out for shipping, not when they arrive at your location
• Shipping times may vary and are not guaranteed
• Refunds are subject to the terms outlined in our Refund Policy
